Salem Free Medical Clinic at TCC

             

Dear Servant Volunteer,

Thank you for your interest in serving at the Salem Free Medical Clinic at Trinity. We hope you will become involved in this wonderful and much needed ministry.

The opening of Trinity’s Clinic is scheduled for the second Saturday in October which is the 10th. All volunteers in the three categories must have a background check, and licensed professionals must have full credentialing.

As you view the three applications on this website, select the application category for you, print and complete it very soon, and mail the application to Salem Free Medical Clinic, PO Box 5095, Salem, OR 97304. Credentialing and background checks take a long time so your submission of your application as quickly as possible is encouraged. All information remains confidential with Salem Free Medical Clinic. I will receive a record of who has submitted an application and in what position that applicant wishes to serve.

If you attend a Church other than Trinity, you are welcome to write your Church name near the Trinity logo on the first page of each application. It will be a pleasure to also have you serve.
In addition to this letter, please read carefully the letter from Marie Howard on this link. You will want her extremely important information regarding training to serve at Salem Free Medical Clinic.
